Who this is between. I am the parent or legal guardian of the child named on this form. I give permission for my child to take part in an AI for All workshop run by Salomante-Dayap Assistance for Humanity and All International (“SDAHA”), a Texas nonprofit.

What the workshop is. AI for All workshops are free lessons about artificial intelligence: what it is, using it safely, spotting AI scams, and using it for school, work and everyday life. Volunteers lead them, in person in Princeton, Texas, or online for the Philippines.

My child’s details. SDAHA keeps the details on this form, with my signature, in a private record that only the two people accountable for its records can open, and uses them only for the workshop. SDAHA never sells them.

Photos. SDAHA takes and uses photos of my child only if I choose Yes below. No is the answer unless I change it.

Changing my mind. I can withdraw this permission, or change my answer about photos, at any time by writing to danilo.salomante@gmail.com or calling (972) 782-4478.
